Our two convenient locations are 2856 Long Beach Road, Oceanside, NY 11572 and 275 N. Central Avenue, Valley Stream, NY 11580.

Please bring your prescription and any notes or reports that describe your condition. You should also bring with you any X-rays, test results, or braces that your doctor may have given you.

For your first visit, please bring the following:

  • Your health insurance card.
  • All copies of medical records and imaging related to any previous treatments for the injury.
  • We will collect payments for the copay, coinsurance, and deductibles when you check in for the appointment.

When going to an appointment, you should wear whatever clothes are most comfortable for you. There is no need for you to dress up in formal attire. T-shirts, shorts, sneakers, are all acceptable to wear.

Physical Medicine and Rehabilitation: During your initial appointment, our team will evaluate the severity of your condition, and discuss your ultimate objectives. We will then develop a treatment plan that is right for you.

Physical Therapy: After going through your medical history, our physical therapists will also conduct several exams to evaluate your physical condition. These tests will gauge several factors including your strength, mobility, and endurance.

Chiropractic: During your initial chiropractic consultation, our chiropractor will first explain chiropractic care and how it can help treat your condition. They will also perform several tests to see which chiropractic technique would benefit you the most.

Physical Medicine and Rehabilitation: The length of a physical medicine and rehabilitation appointment will depend on the severity of the injury or condition. Most people can expect the appointment to last 45 minutes.

Physical Therapy: Your first physical therapy evaluation and session can take anywhere between one and two hours. Treatment sessions moving forward will take 40-60 minutes.

Chiropractic: While the length of the initial consultation will vary from person to person, the first chiropractic session will usually last around 30 minutes.

Please give us at least 48 hours notice if you cannot make your scheduled appointment. By giving us a warning, we will be able to give your appointment time to another patient.
